ARRIVING & CHECKING IN
Check in
When you arrive for your appointment, please check in at the
front desk. You will be asked to verify your insurance coverage and other
demographic information which includes your address, phone numbers, insurance
information, and any other special billing instructions. If you have had a
change in your insurance since your last visit please give our receptionist your
new insurance card so they can scan your new insurance card into our system and
your billing will not be delayed. We understand this can be an inconvenience but
it is best way to ensure we bill your insurance accurately and promptly. If you
have an outstanding past due balance on your account, you will be asked to pay
the past due amount before your appointment.
Please bring the following to each appointment:
- Current insurance card
- Co-pays required by your insurance (these will be
collected before your appointment
- Deposits required by NWPC before you can be seen.
- Identification
- A list of any medication you are taking including
prescription, over the counter and herbal drugs or supplements you are
taking.
Northwest Primary Care accepts cash, check, VISA, MasterCard,
and Discover. Payments may be made at anyone of the Northwest Primary Care
locations, over the phone or by mail.
New Patients
New patients are welcome at Northwest Primary Care!
If you have a valid insurance, you will be asked to pay your
copayment at the time of your first visit. If you are not covered by insurance,
you will be asked to make a deposit when checking in for your first visit. Our
scheduling department will let you know how much you will be asked to pay when
you schedule your first visit.
The charges for your visit are
determined by the services your Practitioner provides to meet your medical needs
and because of the variety of services which may be provided, we cannot always
accurately predict the charges when we schedule your appointment.
The deposit you are asked to pay for your first visit is based on an
estimate of the visit cost. You will receive a statement if your charges exceed
that estimate.
Credit & Payment Policy
New Patient Registration
New patients are asked to complete a
registration form prior to their first visit. Our scheduling department will
mail you the forms that need to completed, if time permits, prior to your first
appointment. If your appointment is scheduled sooner than seven business days, you will
be asked to complete the forms on the day of your appointment. Please arrive
15 minutes early to insure you have time to complete the forms. This information
will be updated annually or whenever a change occurs regarding your registration

New Patient Identification Requirements
New patients must bring a valid
government issued photo identification (such as a driver’s license or DMV
identification card) to receive services at NWPC as well as updated proof of
insurance coverage.
Parking
Milwaukie Family Practice:
Patient parking is located at the East end of the building. Parking along Monroe Street is also available.
Handicapped parking is available on the West side of the building (near Highway
224).
Please don't park alongside any painted curb areas as these
are reserved for ambulance, taxi, and special transport for handicapped
patients.
Talbert
Center
Family Practice: Parking is available in front of our office as well as
on the West side of the building. Handicapped parking is available in the front.
Clackamas Internal Medicine:
Parking is available on the North side of the building. Additional parking is
available in the rear, and can be accessed by taking the entrance to the South
of the building. Handicap parking is available in both lots.
Sellwood:
Patient parking is available in front of our office.
Handicapped parking is available near the front door.
