We are pleased to announce that Northwest Primary Care has been chosen by PEBB and Providence Health Plan to be an in-plan participating provider in the PEBB Choice Plan, effective January 1, 2011.

To be selected as a participating provider in the PEBB Choice Plan, Northwest Primary Care was required to meet "quality of care" capabilities designed to ensure patients receive the best possible health care. We are very proud to have been one of only three medical groups in the Portland area to be chosen.

As a member of PEBB, you may enroll in the PEBB Providence Choice plan or PEBB Statewide Plan October 1, 2010 through December 31, 2010.

PEBB Choice Plan
If you choose this plan, you:

• Will need to select a Medical Home Primary Care Provider (this can be your current NWPC Primary Health Care Provider).
• Will enjoy lower out-of-pocket expenses than if you choose the PEBB Statewide plan.

PEBB Statewide Plan
If you choose this plan, you:

• Can go to any Providence-contracted group.
• Will have significantly higher out-of-pocket expenses than if you choose PEBB Choice Plan
