DISEASE MANAGEMENT
Many of the patients we see have a chronic disease, such as diabetes, asthma, or congestive heart failure. It is our philosophy that the more a patient knows about their disease and how to control it, the better their health and their quality of life. We offer many resources for patients to self-manage their condition, including but not limited to:
Diabetes
For those who enjoy learning in a group setting, we offer a series of classes designed for the patient, as well as their loved ones. The classes cover a variety of topics relevant to the diabetic patient, including how diabetes affects the body, medications, nutrition, self-care, sick days, and more. These classes may be covered by your insurance company.
If you prefer one-on-one type counseling, you may schedule an appointment with our Diabetes Nurse or our Nutrition Specialist. Our Diabetes Team is here to support you in achieving the highest quality of life while controlling your disease.
Asthma
NWPC has an asthma educator who will meet with you on an individual basis to be sure you have created an action plan for your symptoms, help to identify triggers to your asthma, and identify and assist with any questions or concerns you have about living well with asthma.
Medical Nutrition Therapy
Did you know that if you have a chronic condition such as diabetes, high blood pressure, high cholesterol, renal disease, gastrointestinal problems, congestive heart failure, or food sensitivities, your insurance company may pay for nutrition therapy?
Our Medical Nutrition Services include an evaluation of your nutritional status, medical data, and current health behaviors. A specialized plan is designed just for you. Follow-up visits include education, dietary and health behavior counseling and strategies to help meet your goals and improve your energy and overall health.
Follow Up Care
Our clinics have systems put in place to help you and your Practitioner be sure you receive appropriate follow up care. You may be contacted by our scheduling department to schedule a follow up appointment according to your Practitioner's care plan.
