The Health Plan Office at Northwest Primary Care is happy to
assist our patients who have referral based insurance plans with any questions
you have about referrals.
If you’re insurance company requires a referral for a
specialist visit, hospitalization, medical equipment or other services it is
your responsibility to obtain one. We will be happy to assist you in the process
and verify the proper paperwork has been submitted. Please remember that
referrals are also required for follow-up visits as well as diagnostic and
surgical procedures.
Please contact our Health Plan Office at 503-353-1294 if you
are covered by Secure Horizons, Providence Medicare, Family Care or Premier Care
and we will initiate the referral process for you.

How long will it take to process my
referral request? A non emergent referral generally takes 7-10 working days
to process.

How will I know when my referral has been
approved? You will be notified by a
letter or a phone call.

soon as you receive notification that your referral has been approved
