Care for Every Neighbor
The Northwest Primary Care Foundation was founded in 2023 around a shared vision of advancing community wellness within the primary service area for Northwest Primary Care. Through a partnership with the Oregon Community Foundation (OCF), the NWPC Foundation provides grants to local 501C3 organizations that are creating a brighter healthcare future in our community.
Since our first clinic opened its doors in 1955, Northwest Primary Care has remained true to its mission of "providing the best care for every patient at every phase of life." We established the Northwest Primary Care Foundation to extend our mission from our clinics and into the heart of the neighborhoods where we live and work. Through a collaborative partnership with the Oregon Community Foundation, the Northwest Primary Care Foundation exists to advance community wellness throughout NWPC's primary service area.
Our Vision
The vision for the NWPC Foundation is informed by the Social Determinants of Health (SDOH), as defined by the US Department of Health and Human Services. While the SDOH can be categorized into five primary domains, the NWPC Foundation will focus on the two areas that align most closely with the mission of NWPC: Healthcare Access and Quality; and Social and Community Improvement.
Our Model
The NWPC Foundation is made possible through our partnership with the Oregon Community Foundation. The Oregon Community Foundation works alongside people and organizations in Oregon to steward their priorities for strategic charitable giving in order to create lasting, transformative change across the state. This partnership enables us to direct 100% of every dollar given to the NWPC Foundation into organizations that are building a healthier community.
