Advance Care Planning Class (In-person & Virtual)

Planning for your future healthcare is an important step toward making sure your preferences are clear if you are unable to speak for yourself, or family members are making medical decisions for you. Advance Care Planning involves considering your desired outcome ahead of time and clearly communicating with your family and healthcare providers. These types of decisions are documented in an Advance Directive. Advance Directives ensure patients avoid unwanted procedures, provide clear guidance to health care providers, and reduce the likelihood of a dispute among family members.

Join us to learn about Advance Directives, why they matter, and how to engage your family with these important conversations. This class is available in-person or via Zoom.

Diabetes Self-Management Class (In-person)

The Diabetes class is designed for patients with a new diagnosis of diabetes, those needing a refresher course, and family members looking to support a loved one with diabetes.

  • Class is $50 per participant
  • Participants may bring guests at $20 per person
  • Registration is required
  • Lunch is provided
  • Open to the public

Great Beginnings Childbirth And Newborn Education Class Series (In-person)

Our Great Beginnings Classes are specifically designed to provide you with lots of tools to help make your last weeks of pregnancy and all the way through labor, delivery, hospital stay, and first few weeks at home as a new family the best experience possible. Class is two sessions and $150 fee covers both sessions. Registration required. Our Childbirth Educators welcome you and your birth partners and look forward to meeting you soon.

This is a two part class.

  • Part 1: September 21, 5:00 - 7:00 PM at Oregon City Clinic
  • Part 2: September 23, 10:00 - 4:00 PM at Oregon City Clinic

Persistent Pain Self-management Class (In-person)

This class will define the types of pain and explore the importance of physical therapy and behavioral health support for managing chronic pain. Insurance will be billed for participants in this class and may result in a copay or co-insurance being billed to the participant. This class is open to Northwest Primary Care patients. Cost is $35.

In person: November 18, 12:00 PM - 5:00 PM. Light refreshments provided!

Alzheimer’s Support Group (In-person)

This monthly support group is open to all who have friends or family with a diagnosis of Alzheimer’s or Dementia. Please join us to connect with others who understand the challenges of caring for a loved one with a progressive illness. This group is free and open to the public, and no RSVP is required.

Where: NWPC Sellwood Clinic

When: 3rd Wednesday of every month at 6:30 PM

Empowerment Entourage Support Group

This monthly support group is meant to be a safe space for new birthgivers to share about the challenges and successes of new parenthood.
