Advance Care Planning (Zoom)

Planning for your future healthcare is an important step toward making sure your preferences are clear if you are unable to speak for yourself, or family members are making medical decisions for you. Advance Care Planning involves considering your desired outcome ahead of time and clearly communicating with your family and healthcare providers. These types of decisions are documented in an Advance Directive. Advance Directives ensure patients avoid unwanted procedures, provide clear guidance to health care providers, and reduce the likelihood of a dispute among family members.

Join us to learn about Advance Directives, why they matter, and how to engage your family with these important conversations.
