If you would like to make an appointment with your care provider or for another service, please call our scheduling team at 503.659.4988.
In order to make your appointment as quickly and easily as possible, please be sure to have the your insurance information, address, and telephone number ready when you call our scheduling team. It is also important to let the scheduler know whether your appointment should be billed to your medical or motor vehicle insurance carrier, or to a worker’s compensation or other third-party insurer, or if you are using our private pay option. If you do not have medical insurance, please ask about our SureCare program, which may offer discounted fees for our services.
Prior to your first visit, we will ask you to complete our registration forms. If your scheduled appointment will take place within three business days, we will ask you to complete the forms on the day of your appointment. If your scheduled appointment will take place after three business days, the necessary forms will be mailed to you to be completed. In order to allow time for paperwork and registration, we request that you arrive for your first appointment 15 minutes early.
Remember to bring proof of your current insurance coverage to your first appointment, as well as a valid government-issued photo identification (such as a driver’s license or DMV identification card), as we may not be able to serve you without valid photo identification.

Checking in for Your Appointment
When you arrive for your appointment, please check in with our receptionist. We will ask you to verify your insurance information, address, telephone number and any special billing instructions. To ensure that we bill your insurance accurately and promptly, if your insurance has changed since your last visit, please give the receptionist your new insurance card to be scanned into our system. If there is an outstanding balance on your account, we will ask that you pay the past due amount before your appointment.
What to bring to your appointment
- Your current insurance card (if applicable)
- Valid photo identification
- Any co-pays required by your insurance (to be collected at check-in)
- Any pre-appointment deposit required by NWPC (if applicable)
- A list of any medications you are currently taking (including prescription and over-the-counter medications, and herbal supplements)
Northwest Primary Care accepts cash, check, VISA, MasterCard and Discover. Payments can be made by telephone or mail, or at any of our locations.
