Your Home for Family Medicine

Family medicine is built on long-term relationships. Over time, your provider develops a deeper understanding of your health history, your priorities, and patterns that help inform care when new concerns arise. This broader perspective can make it easier to support your health for the long haul.

That is what family medicine at Northwest Primary Care is built around: a provider who knows you, understands your health over time, and partners with you in caring for both current needs and future wellbeing.

What Is Family Medicine?

Family medicine is the broadest branch of primary care. Family medicine physicians are trained to care for patients at every stage of life, from newborn care and infancy through adolescence, adulthood, and into the senior years.

That range is intentional. A family medicine provider who has cared for a parent for a decade brings real insight when that parent's adult child becomes a patient. A provider who knew you in your thirties is better equipped to manage your health in your fifties. The longer the relationship, the more useful the history.

Care for Your Whole Family, Under One Roof

One of the practical advantages of family medicine is that a single provider can care for every member of your household. Parents and children, grandparents and teenagers all with the same provider, the same clinic.

Beyond convenience, this shared perspective can also support better care. A provider who knows a parent's history has meaningful context when a child presents with similar symptoms. When health patterns run in families (as they so often do), a provider who sees the whole picture is better positioned to act on it.

Caring for multi-generational families across the Portland area has been central to Northwest Primary Care for decades. It reflects the kind of long-term relationships many of our patients value most.

What Makes NWPC Family Medicine Different

Long-term relationships as the standard, not the exception

It's not uncommon to see patient-provider relationships of 20 years or more at NWPC. This is because our practice is structured to support continuity: the same providers, stable over time, in a physician-owned practice where the focus is on patient care rather than patient volume.

On-site services that stay connected

When your family medicine provider refers you for physical therapy, a sleep study, labs, or a mammogram, those services are available within NWPC. Results go directly into your shared electronic health record and are available on your patient portal. You do not coordinate between separate offices. Your provider already has the information when you come in for your follow-up.

A physician-owned practice

NWPC is owned by the physicians who practice here. That independence keeps the focus where it belongs: on providing the best care for you and your family. It also allows our providers to make decisions based on what supports patients, not outside priorities.

Preventive Care and Annual Wellness

Family medicine is, at its core, preventive. This means the annual exam is not just a formality. It is the visit where your provider checks what has changed, makes sure you are up-to-date with recommended screenings, and catches any new issues before they become harder to manage.

Blood pressure that has crept up. Cholesterol numbers worth watching. A family history that may warrant regular screening. These are the things that routine preventive care finds, and that caught early, rarely become the serious problems they might otherwise.

Your NWPC family medicine provider will work with you on a preventive care plan that fits your age, your health history, and your specific risk factors. That plan evolves as you do.

Managing Chronic Conditions Over Time

For patients diagnosed with ongoing conditions, like diabetes, high blood pressure, asthma, thyroid disease, high cholesterol, and others, consistent primary care is where long-term management happens. Your provider tracks your test results across visits, adjusts treatment as needed, coordinates any specialist involvement, and makes sure nothing falls through the gaps between appointments.

This is one of the clearest areas where long-term relationships can produce better outcomes. A provider who has watched your health trends for years has a fundamentally different picture than one seeing you for the first time. Learn more about how NWPC approaches chronic disease management.

Same-Day Visits

When health concerns arise, our Family Medicine team is here for patients of all ages. Urgent care centers and emergency rooms can often involve higher costs and long wait times, so we encourage you to contact us first for non-life-threatening medical needs. Call our scheduling team at 503.659.4988, and we will work to arrange an appointment as quickly as possible.

If you are an established patient, we will make every effort to schedule your same-day visit with your primary care provider. If that is not available, we will help you be seen as soon as possible at the clinic location most convenient for your family.
