Geriatric care is the planning and coordination of care for an elder patient to meet their long term needs, improve their quality of life, and if possible, help them maintain their independence for as long as possible. Since there are many factors to consider, your practitioner and care coordinator may conduct a care plan assessment to identify your unique needs. Our geriatric care coordinators work in conjunction with our practitioners and are available in each of our clinics whenever you need assistance.
